Which ‘Best Medicine’ Stars Are — And Aren’t — Returning for Season 2 After Cast Changes?

Best Medicine is returning for season 2 on Fox — but is every cast member coming back to the show?

The show follows a doctor who moves from Boston to a small practice in the East Coast fishing village where he vacationed as a child. In addition to Josh Charles, the cast includes Josh Segarra, Abigail Spencer, Cree Cicchino, Annie Potts, Didi Conn, Clea Lewis, Stephen Spinella, Jason Veasey, Cindy De La Cruz, John DiMaggio, Carter Shimp and dog Wattson.

“The way that [executive producer] Liz Tuccillo has structured the show, every episode we have a great star come on and they get to do something delightful — and maybe something you’ve never seen them do before,” Spencer exclusively told Us in January 2026. “I would say what I particularly loved about the show was that it is a great opportunity for some of our more seasoned actors in the community.”

She continued: “There’s a lot of roles across ages where I’m like, ‘God, we could get so many great actors to come.’ I loved that.”

Josh Charles

There would be no Best Medicine without Charles’ performance as Martin Best.

Abigail Spencer

Spencer is front and center in season 2 of the hit medical drama.

Josh Segarra

Segarra, who plays Mark Mylow, is back in the role of the town’s sheriff and Louisa’s ex-fiancé.

Cree

Elaine’s journey continues in season 2 of Best Medicine.

Annie Potts

Potts will continue playing Aunt Sarah a.k.a Dr. Best’s aunt and the local lobster woman.

Stephen Spinella

Spinella was promoted for season 2 after introducing Us to Greg.

Jason Veasey

Veasey’s role as George has also been expanded in season 2 of Best Medicine.

Carter Shimp

Elaine’s love interest — Al — will be featured in the sophomore season.

Wattson

After being named Copernicus, the town’s stray dog is coming back in season 2 as Dr. Best’s pet.
