What Happens When You Stop TRT? Recovery Timelines, Side Effects and Fertility Risks for Men

For readers who treat health as a daily practice, stopping testosterone replacement therapy is a planned medical decision, not something to improvise. Feel30, a telehealth provider specializing in men’s health, recorded an estimated 34,830 U.S. searches across 16 distinct query terms about treatment duration and discontinuation in the 12-month period ending August 2026, with “coming off TRT after 6 months” alone drawing roughly 6,700 searches.

What Happens to Your Body When You Stop TRT?

Testosterone levels typically fall back to pre-treatment baseline and the original symptoms can return, which is why doctors warn against stopping abruptly. According to Gatlan, a telehealth program, “abruptly stopping TRT is not recommended. It can cause significant health risks and unpleasant withdrawal symptoms.” The site lists fatigue, mood changes, hot flashes, trouble sleeping, erectile dysfunction, weight gain and bone loss as possible side effects.

Mike Bohl, MD, a general practitioner and Senior Director of Medical Content & Authority at Hims & Hers, told Hims, “When you go off TRT, you’ll lose the benefits it provided. Your testosterone levels will go back to your baseline, and your symptoms may come back. It can also take several months to more than a year to see a full return of your baseline hormone production.”

How Long Does a Safe TRT Taper Take?

Recovery varies widely, with some men bouncing back in weeks and others needing six to 12 months or longer. Hims notes that timelines depend on age, how long someone has been on TRT and other health factors, and that blood tests and provider supervision are necessary to track recovery and manage symptoms.

Gatlan recommends gradually reducing the dose over several weeks or months, saying the approach allows the body to adjust, reduces withdrawal symptoms and supports a return to natural production. TRT Nation adds that natural production restarts gradually and that men should expect physical shifts in energy and muscle mass along with possible emotional adjustments.

Does Stopping TRT Affect Male Fertility?

Yes. Discontinuing TRT can reduce sperm production because the therapy suppresses key reproductive hormones, including luteinizing hormone (LH) and follicle-stimulating hormone (FSH). Restoring baseline sperm counts typically takes six to nine months or longer. Enclomiphene or human chorionic gonadotropin (hCG) may be considered to maintain testosterone without compromising fertility.

A 2016 study of 262 hypogonadal patients, mean age 59.5, who received testosterone undecanoate at 12-week intervals for up to 11 years found that interrupting treatment pushed total testosterone back to hypogonadal levels and worsened obesity measures, erectile function scores and prostate symptoms until therapy resumed. Researchers concluded that “hypogonadism may require lifelong TRT.”

What Is Post-Cycle Therapy and How Does HCG Help After TRT?

Post-cycle therapy (PCT) uses medications like Clomiphene and HCG to reactivate the hypothalamic-pituitary-testicular (HPTA) axis, which TRT temporarily suppresses. Denver Regenerative Medicine explains that stopping without PCT leaves the body in a temporary state of hypogonadism, with external testosterone gone but natural production not yet restarted. Common SERMs include Clomiphene Citrate (Clomid), typically dosed at 25 to 50 mg daily for four to six weeks, and Enclomiphene at 12.5 to 25 mg daily.

HCG mimics luteinizing hormone, telling the testes to produce testosterone and sperm. Gameday Men’s Health says HCG can ease fatigue, low libido and testicular atrophy, and is particularly valuable for men hoping to preserve or restore fertility, though it should only be used under medical supervision with regular hormone monitoring.
