Exclusive | We got filler in our balls — how it can combat a sneaky TRT side effect and help in the bedroom

After being on testosterone replacement therapy (TRT) for several years, 45-year-old Miguel Anda noticed a common but less-than-aesthetically pleasing side effect — smaller testicles.

While he wasn’t initially self-conscious about it, he eventually decided that the family jewels should match the rest of his package.

“I use the word insecurity because I feel like… when it comes to men’s balls, we want them to look hefty,” the professional DJ and former adult film star told The Post.

The solution Anda decided on was ScroFill, a non-surgical procedure that injects hyaluronic acid filler into the scrotum to make it appear larger, fuller and heavier.

“I’m not getting new balls,” he added. “It’s kind of giving the illusion of a big sack… people think: big sack, big balls.”

Anda was one of the first patients at Lushful Aesthetics, a men’s med spa that’s been cheekily referred to as the “Bulge Clinic” — because it offers scrotal enhancement as well as a similar procedure known as GirthFill, which adds width, but not length, to the penis with filler.

Many patients end up getting both procedures done — either at the same time, or ScroFill after GirthFill to balance out their bulge.

Despite the needles, Anda insists it wasn’t painful — a topical numbing agent and local anesthetic help — but it certainly looks painful in the immediate aftermath.

“When you initially see, it looks like a bunch of bees stung you,” he explained. And then there’s the aftercare. “You have to play with your balls and knead them out so that [the filler is] evenly distributed.”

But the magic happens soon after the initial injections.

“Then in a couple weeks… it’s heavy and hefty and evenly distributed filler throughout your sack,” Anda said.

Who’s getting the total package

Chris Bustamante, an aesthetic nurse practitioner and the founder of Lushful Aesthetics, says men mainly come for the same reason: restoring confidence and body image.

The biggest misconception is that only one ‘type’ of man seeks these treatments,” he said. “In reality, patients are diverse and motivated by confidence, self-image and feeling comfortable in their body. “

He treats patients from all walks of life: Blue-collar and white-collar workers, a 50-50 split between bisexual or gay and heterosexual men, and ages as young as 30 years old and up to 65.

The amount of hyaluronic acid filler needed varies by person, but most first-time patients use 10 to 15 syringes, with the first 10 syringes costing $7,500 and each additional syringe costing an extra $600 per syringe.

He also says that while some patients are men who are affected by hormone or age-related atrophy or were born with a smaller penis or balls, more often those who end up at his clinic are already average or above-average men looking for a bigger package.

Bigger and better — in bed

The common denominator among many of the guys who want to alter their “bulge size” is restoring confidence in the bedroom.

Though these procedures are cosmetic and not intended for treating sexual dysfunction, Bustamante said they have neither a negative nor positive effect on sexual performance — but feeling good about yourself does.

“Many patients report increased confidence during intimacy, less self-consciousness in general — and their partners love it,” he said.

“It’s really, really cool to see that people are noticing, and… people’s eye gaze had changed.”

“I’ve had several patients tell me their wives are very satisfied with their newly added girth,” he went on. “Funny enough, many of my straight patients have said their wives encouraged their interest in getting GirthFill.”

Beating the TRT shrink

That was the case for Brandon, another of Bustamante’s patients. Brandon had been on TRT for a year, and his confidence during sex took a hit after he and his girlfriend noticed testicular shrinkage.

With support from his partner, he got procedures last November.

He said there was only “some mild tenderness” afterwards — and both injections ended up having a more positive impact than he thought they would.

“[It] both definitely helped with my confidence,” the 38-year-old ICU nurse told said. “It definitely compensated for any volume loss that was associated with the hormone therapy.”

His girlfriend also noticed and was “very satisfied” with the change too, he noted.

A fun way to treat himself

Carl, another patient, saw the below-the-belt tweaks as a bit of “fun.”

“It can be something fun and just for your own sexual pleasure,” the Manhattan-based cybersecurity professional said. “This is something fun you can do for yourself and just feel even more confident than you already are.”

He got GirthFill in March 2025 and thought Bustamante “made it feel fun and casual and more about embracing your own sexuality.”

He was a bit worried about how long it would take and the recovery process — but all said and done, he realized he didn’t need to be.

“It felt like nothing,” the 38-year-old said. “It was so easy and convenient. Like I literally just came in on a longer lunch break. We did the procedure, we wrapped, put them bandages on and I was back on a call by four.”

After receiving compliments from partners about his size post-procedure, Carl made the immediate decision to go ahead with ScroFill as well.

“It was super fun to start getting like comments that I normally wouldn’t have received,” he said, adding that he’s noticed a difference when both erect and flaccid. “It’s really, really cool to see that people are noticing, and… people’s eye gaze had changed.”

All three men have said they’ll be returning for maintenance touch-ups. Though results fade away after two to three years, a re-up is recommended every 12 to 18 months.

For Brandon, the results are worth continuing to head back to the clinic.

“I think the main motivation for this was just about restoring what the TRT therapy took away from me, and feeling balanced,” he said. “And all of the results for everything [Chris] did were very natural, and it definitely helped with my confidence and my relationship.”
